Brain Hemispheric Questionnaires

For an accurate assessment of your child’s brain function, please complete all 200 questions. There are 100 questions for the left brain delay, and 100 questions for the right brain delay. The whole questionnaire should take 15-20 minutes to complete. If the question is more neutral, then simply pick YES or NO for that answer and we can discuss that particular question. The purpose of this questionnaire is to find out where the brain delay exists. Please do not omit any question.
